CLIENT AGREEMENT

I'm super excited to start our work together. Before we do, I have some admin that I will need to you complete. It should take no more than 30 mintues, 

First please complete the Client Agreement Form, this provides me with information about you prior to our work together and our working relationship. Everything is treated in complete confidentiality.  

After if you can then make your payment, either in full or via payment plan as agreed. I use Stripe as my main payment gateway but I can accept payment via paypal. Please email me for details. 

Please can you ensure this is completed at least 24 hours before our first session so that we can maximise our time together.
